PRE-REGISTRATION FORM:

The following PRE-REGISTRATION FORM will only submit the player application information into our database (if your child played Fall Ball 2009 you do not need to PRE-REGISTER). You will still be required to bring your PROOF OF RESIDENCY and CHILD'S BIRTH CERTIFICATE on a scheduled registration day in order to complete the process. Failure to do so may result in your inability to register.

Additionally, we do not accept payment until PROOF OF RESIDENCY and your CHILD'S BIRTH CERTIFICATE have been verified by a registrar.
